Choice and eligibility
Our contact form offers two separate, optional, unchecked-by-default consent choices: non-marketing calls and SMS, and marketing calls and SMS. Consent is not a condition of purchase, receiving services or submitting an inquiry. Enrollment in either calls/SMS category is limited to adults age 18 and older. The practice can provide care to patients of other ages; a parent or guardian may contact us about a minor’s care without enrolling the minor in the communications program.

Program and enrollment
Quantum Medical & Wellness Center (Integrated Medical Centers Inc.) offers optional recurring non-marketing calls and SMS about appointment confirmations, reminders, inquiry responses and scheduling updates. Separately opted-in marketing communications cover special offers, promotional practice news, and general wellness education. Select one or both consent categories on our Contact Us page to enroll. A form submission without either selection does not enroll you. You must be age 18 or older to enroll, provide a number you are authorized to use, and keep your contact details current.
Frequency and costs
Call frequency varies. SMS message frequency varies, up to 10 messages per month total across all categories you select; this is a combined monthly limit, not 10 per category. Message and data rates may apply according to your wireless plan. Contact your carrier about your plan.
Stopping calls or SMS
Reply STOP to any program SMS to opt out of SMS. You may receive a final opt-out confirmation text. To stop phone calls, tell our representative during a call or contact (954) 370-1900 or [email protected]. You may withdraw consent for calls, SMS, or both. The SMS STOP keyword does not stop phone calls. We maintain your opt-out preferences to honor your request.

Delivery and scheduling
SMS delivery depends on wireless carrier availability and other technical conditions. Carriers are not liable for delayed or undelivered messages. Appointment requests are subject to availability and confirmation by the practice. Messages are not an emergency or medical advice service.
